Community Link Workers and Health Trainers provide tailored, one-to-one support for vulnerable older people to enable and empower them to live independently in the community.
Community Link Workers
Guild Care has six Community Link Workers based at the Healthy Living Centre, who are part of the Worthing and Adur Community Partnership Team (CPT); a multi agency, multi disciplinary team funded by the county’s Partnerships for Older People Project (POPP).
Community Link Workers offer short term practical support, information and encouragement to help you improve your health, general wellbeing and access benefits.
They support an average of 250 older people during a year by acting as a link between different services and can refer or signpost older people to other sources of support.
Health Trainers
Health Trainers support people over the age of 60 who would like to make a positive lifestyle change by improving their health and wellbeing. This may include diet, exercise, confidence building, reduce smoking and alcohol consumption, improve mental wellbeing and addressing social isolation.
They help people focus on their achievements encouraging motivation and confidence to improve wellbeing and maintain a healthier way of life.
Referrals can be made by family, friends and health professionals or you can refer yourself. If you are referring someone, please ensure you have their permission to do so.
