Working with people in the community
Our caring and experienced Prevention Link Workers, based at the Guild Care Centre (Methold House), work with older people in the community to offer practical support, confidence building, information and encouragement to help improve general wellbeing and offer better access benefits. Referrals can be made by family, friends and health professionals or you can refer yourself.
They can make home visits and also deliver services in community settings with one-to-one short term packages to help improve the quality of your life. This might include ~
Home adaption and equipment
Help to access appropriate benefits
Social clubs and activities
Taking more exercise
Keep warm and secure
The team has strong links with other health and wellbeing initiatives and are experienced in working to support older people. They arrange a number of small gatherings of people, helping those who may have become isolated to make new friends. These events could be a 'Tea & Chat' with a quiz or a Tai Chi session locally.
