Bank Take Home and Settle Support Worker - (5 years fixed term funded role) ***Candidates must live in a 30-minute radius of Worthing Hospital** We are looking to recruit a Take Home and Settle Support Worker to work on a bank basis (as and when required). You will provide support for vulnerable adults who have attended A&E to go home if they are able, rather than being admitted to hospital because there is no support at home. You will also provide support for those who have had a stay in hospital and are ready to be discharged safely back into their own homes. You will assess the customers overall situation and ensure that other areas linked with their wellbeing are also being addressed and if necessary, make referrals to other agencies, including to colleagues in Community Services. You will be responsible for risk assessing the property of the customer to ensure that they are safe to stay, make small changes if needed, and report to the Community services manager if further safety issues need addressing. You will focus on delivering a good quality person-centered service for customers. It is essential you have experience of working with vulnerable adults and knowledge of health & prevention issues for older people. A good understanding of the value of low-level intervention in promoting wellbeing and a good coping mechanism to manage your own emotional response to the work are a must. Due to the nature of the role, you will need to be within a 30-minute commute from Worthing hospital during working hours.
