As this is a role that involves directly caring for our vulnerable clients, residents, and family members, you will need to provide evidence that you have been vaccinated against Covid, before starting your new role as per our company policy. We are looking for a Community Co-ordinator to support older people to access and engage with an extensive range of support and activities in the local community to help to reduce social isolation and improve their health and wellbeing.

You will be expected to co-design services with and for older people to identify appropriate support to meet their needs and improve individual outcomes around reducing isolation, enhancing health and wellbeing and maximising independence.

You will ensure the Creating Connections Service is effectively promoted to maximise the number of beneficiaries.
