We are looking for a Home Care Coordinator to manage a team of Home Care workers, ensuring the day to day delivery of the Home Care Service in an agreed area. You will support people living with complex needs to live well, safely and have fulfilled lives, in a way that is focused on the individual and is based on what matters to them. You will be expected to work flexibly with people in a way that continually demonstrates a focus on being person-centred and relationship focused.

We would like you to bring commitment, integrity, passion and the enthusiasm to the role that people using our service deserve. We want you to be yourself, have a sense of fun and take ownership for all aspects of your role. It is essential you have proven experience of supporting older people, including those with dementia to live independently.

You must be able to lead a team of staff, communicate with a range of different people and have good IT skills. Good knowledge of the local area and ability to travel throughout the area is a must. 

You will be expected to participate in an out of hours on call rota acting as first point of contact for frontline staff and clients and providing appropriate advice and guidance in an emergency, which includes weekends and evening.
