£21,226- £23,129 per annum (£11.66- £12.71 per hour).

On call paid at £9.90 per hour

Hours of Work

35 hours per week

Week 1= 7:00am-3:00pm, Week 2= 2:30pm- 10:30pm rotating with another coordinator. Monday- Friday

On call will be one weekend in three – 7:00am -10.30pm Saturday and Sunday

Guild Care's Home Care service was established in 1994 as part of its mission to promote independent living. Our team are a strong tight-knit group covering a small geographical area with a dedicated team of caring and professional Community Care Assistants providing services such as personal care, domestic help, emotional support, and general assistance with other tasks, such as shopping, medication, and mobility. Older people and vulnerable adults in Worthing can live a better, more comfortable life at home with the help of this service.

The Home Care team are seeking a Care Coordinator to supervise a team of Home Care Workers to ensure the seamless delivery of the service from day-to-day. You will support people living with complex needs to live well, safely and have fulfilled lives. You will ensure that the service provides support and care in a way that is focused on the individual and is based on what matters to them.

This role calls for the ability to work in a flexible manner with people and to continually demonstrate a commitment to being person-centred and relationship focused. You will be expected to assess the needs and risks of new and existing clients to develop person-centred care plans that are customised for everyone. In your role, you will be expected to recognise when the individual's needs change and to support them in obtaining the appropriate services. Because of this, you will need to work flexibly, adapting your work schedule to meet the needs of the service.

By taking charge of rostering for the staff in your area, you will ensure all clients receive their care, considering the individual needs and preferences of each client. You will also be responsible for the supervision and appraisals of staff. To keep accurate and up-to-date records for clients and your staff, you will maintain electronic and handwritten records. We expect you to possess a high level of reliability, commitment, integrity, passion, and enthusiasm to fulfil the role, whilst being yourself, having fun, and taking responsibility for all aspects of your responsibilities.

Additionally, you will be expected to participate in an out-of-hours on call rota serving as the primary point of contact for frontline staff and clients and offering advice and guidance as necessary.

Job description


  • Experience of working in the care sector
  • Experience within domiciliary setting
  • Proven experience and a passion for supporting older people, including those with dementia to live independently
  • Ability to lead a team of staff
  • Ability to communicate with a range of different people
  • Good IT, literacy and numeracy skills
  • Good knowledge of the local area and ability to travel throughout the area
  • Ability to participate in an out of hours on call rota
  • Ability to provide care to clients in an emergency
  • Satisfactory DBS check


  • Experience of assessing the needs and risks of vulnerable adults
  • Experience of rostering staff and managing rotas
  • Experience of managing a team and completing staff supervisions and appraisals
  • Understanding of mental capacity and deprivation of liberty
  • Previous experience of using ICARE (business manager experience for rostering)
