Home from Hospital

This service is fully operational throughout the coronavirus pandemic. If you have any queries, please contact our friendly Customer Service Team on 01903 327327. 

Will you be going into hospital soon? Are you worried about coping when you return home? Our Home from Hospital service can help you manage.

The Home from Hospital service helps people living in West Sussex to remain independent at home following a stay in hospital.  

What is provided?

Up to six weeks practical support after discharge from hospital for local residents in West Sussex.

Guild Care is providing the service for residents in the Adur and Worthing areas. We work in partnership with Age UK West Sussex and Age UK Horsham provide the service in other parts of West Sussex. 

The service supports

  • Older people
  • Vulnerable adults
  • People living alone
  • Anyone about to leave hospital or who has recently been discharged

How we can help

  • We will visit and make sure you are safe, have what you need and that the relevant services are in place should you need them.
  • We will ensure you are not isolated following your return home from hospital.
  • We will visit to help you regain your confidence and independence.

Even if you have already made arrangements such as home help or personal care,  our 'Home from Hospital' volunteer could:

  • Offer social and practical support
  • Collect prescriptions and basic shopping
  • Prepare a light snack
  • We may at times and where appropriate, support you to attend follow up hospital appointments
  • Provide information on other services or equipment you might need
  • Pop in for a chat
  • Telephone support
  • Escorted walks
  • Engaging in local activities
  • Signposting to other services

Please note that this service does not offer personal care, nursing/clinical care, dispensing medication, counselling, or domestic cleaning.

My volunteer was polite and had a lovely Irish accent. You would be surprised how many jobs she did for me and still had time for a lovely chat. I would certainly recommend the service.
